5- Bill Adama (Battlestar Galactica) that "do not mistake this for approval" icon is brilliantly accurate. Nothing is ever good enough for Adama, and everything is always about how he feels.
4- Gendo Ikari (Evangelion) They've named the fictional award to bad father for him for a reason.
3- Ozai (Avatar the Last Airbender) to Azula as much as to Zuko
2- Tywin Lannister (A Song of Ice and Fire) for Tyrion and for Cersei especially, though I liked how the TV show high lightened to wasn't exactly perfect with Jaime either.
1- Gerald Tarrant (Coldfire trilogy) the most horrible thing is, he was probably a good father, until he, you know, murdered his children.

That does sound pretty bad.
How much is character derailment compared to stuff that are true to the character?

It's Slade. It's not like he doesn't do mindgames. Sometimes he does mindgames by not doing mindgames. At times he's presented as more of an anti-hero than an all-black villain, which works for me.
Nuking Blüdhaven was OOC for Deathstroke. The stuff with Rose?... Debatable. He did get her one of the best teachers around.
